Not the one that ran a little long, or the one released after a documented justification. The one that failed in a way that showed what the system had really become. This is the story of that batch, and what it taught our team.

What a batch actually is

A batch is not just a quantity of medicine. It is a documented unit of manufacture: specific materials, a specific formula, specific equipment, a defined period, and a set of in-process and quality control checks that together prove the product meets its specification.

So when a batch fails, it does not simply mean the product is wrong. It means a link in that chain of evidence has broken. What follows is fixed: investigation, documentation, root cause analysis, corrective and preventive action (CAPA) and, in most cases, rejection of the batch. A failed batch is not proof that a system is broken. It is the system working, bringing into view something that was always there.

The Monday morning call

It began with a call from QC on a Monday morning. The batch was a cytotoxic oncology formulation, made for a client we had worked with for more than a year. On paper it had been a clean run: no in-process deviations, every parameter within range.

The finished product assay told a different story. The API content was below the lower limit of the specification. Not catastrophically, but clearly, and on record.

For an oncology drug, that matters far more than the number suggests. A subpotent product may fail to reach therapeutic levels in a patient whose treatment window is measured in weeks. It is not a technical non-conformance. It is potential patient harm. The batch was put on hold immediately.

What our Managing Director remembers most is not the result, but the silence on the production floor, and the look on the face of a supervisor with fifteen years in manufacturing. Not panic. Gravity.

Eleven days, three calls

The investigation took eleven days. In that time the client called three times. The first call was informational, made within hours of the QC result, as our protocol and our relationship required. The second came three days later, asking for a timeline. The third came on day seven, with an urgency we understood completely and could not accommodate.

Inside the company, the pressure to find a reason to release the batch was real. We say so openly, because a quality story in which holding a batch is easy and everyone agrees is not a true story. QA held the batch not because they were rigid, and not because they lacked commercial awareness. They held it because the assay result was real, the investigation had found no testing error, and releasing subpotent oncology product is not a commercial risk. It is a patient risk.

What the investigation found

The root cause was both specific and humbling. It traced back to the blending stage. The blend time in that batch had been logged as within the acceptable range, but right at its very edge. When we modelled how an edge-of-range blend time affects the spread of API through the granules, the result matched the low assay.

Going further back, the blending range in our SOP had been set during the original process validation, on equipment slightly different from what was running now. The blender had been serviced and reassembled during a maintenance cycle, and a subtle change in mixing dynamics was not captured when it was re-qualified.

No single step was a careless mistake. Each was defensible on its own. Together they left a gap, and the batch fell through it. Root causes are almost never isolated. They are usually a confluence of individually defensible factors that add up to an unacceptable outcome.

What the team carries after a failure

The batch was formally rejected on day eleven. The client received a full written summary of the investigation findings, the root cause and our CAPA commitments, and a replacement batch was scheduled immediately.

What we had not fully expected was the emotional weight on the production team. The operators who ran that batch knew they had not been careless. They had followed the process as they understood it, and the failure had not come from inattention or negligence.

The worst thing a leader can do after a batch failure is look for a person to blame. We held a full team debrief instead. It was not a disciplinary meeting, but a genuine sharing of the investigation. The operators who ran the batch were given the dignity of understanding that the failure was systemic, not personal. How leadership treats a team after a failure shapes how that team performs in the next crisis, and accountability without blame is a skill worth practising on purpose.

The replacement batch

The replacement batch ran three weeks after the rejection, and it ran cleanly. Not through luck, but because the system was different. The blender had been re-qualified, the SOP had been revised, and the operators understood why the parameter limits sat at their new values.

The client received it on schedule, with a full quality package that included the CAPA summary. They did not reduce the relationship. They deepened it.

What buyers can take from this

If you are choosing a manufacturer, especially for oncology or any high-risk product, these three questions tell you a great deal:

Ask

A good answer looks like

What happens in the first hours after an out-of-specification result?

The batch is held at once, and the client is told within hours, not days.

Can you show a CAPA summary from a past failure?

A written investigation, a root cause, and the changes that followed.

After equipment is serviced, how do you confirm the validated parameters still hold?

Re-qualification that covers how the equipment actually behaves, not only that it runs.

A manufacturer who says they have never had a failure has either not looked closely or is not being honest. The question is not whether failures happen. It is what the company does next.

What this means for everyone else

If you take medicines, a rejected batch is not a scandal. It is a safety net doing its job. The failure to worry about is the one that gets quietly released. Behind every tablet that reaches a pharmacy shelf is a record of checks, and a team willing to stop the line when the record does not add up.

How Salus approaches this

Salus Pharmaceuticals has manufactured formulations at its WHO-GMP and ISO 9001:2015 certified facility in Baddi, Himachal Pradesh, since 2005. Our rule for a quality event is the one this chapter ends on: minimise nothing, explain everything, fix it visibly.

Buyers and importers are welcome to ask us how we handle deviations, investigations and CAPA before they place an order

. Send an enquiry →

This story is taken from our book

Read more about “Killing Cancer” →

Every pharmaceutical manufacturer has a batch they do not talk about easily.
